Insurance

We accept health plans from most of the major insurance providers.

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

We recommend asking these questions to your insurance provider to help determine your benefits:

  • Does my health insurance plan include mental health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for services to be covered?

We currently accept the following plans:

  • Aetna (SC, NC, GA, FL)

  • Blue Cross Blue Shield (SC ONLY)

  • Cigna (SC, NC, GA, FL)

  • United Healthcare (SC, NC, GA, FL)

  • First Sun EAP

  • Anthem BCBS EAP

  • Spring Health EAP (SC, NC, GA, FL)

  • Lyra EAP (SC, NC, GA, FL)

  • Espyr EAP

  • TRICARE (SC Out of Network Only)

Don’t see your plan on our coverage list? Call L.I.F.E. with KB, LLC today and we will do our best to accommodate your situation or provide coverage answers.

Good Faith Estimate

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as testing, prescription drugs, etc. Your health care provider shall provide you a Good Faith Estimate in writing prior to your medical service. You can also ask your health care provider and any other provider you choose to work with for a Good Faith Estimate during scheduling. If you receive a bill that is substantially higher than estimated on (more than $400 than) your Good Faith Estimate, you can dispute the bill. It is a good idea to save a copy of your Good Faith Estimate. Your Good Faith Estimate will be given to you upon intake with the intake specialist and prior to starting counseling, medication management, or testing services. Please visit our service page for more information on services that are offered in our practice. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
